Blended Case Management

Adult Blended Case Managers assist individuals who are diagnosed with a mental illness in gaining access to available community resources.  These resources include, but are not limited to:  accessing benefits, securing housing, employment opportunities, advancement in education, and opportunities for increasing socialization and decreasing isolation.  


The Blended Case Manager serves as an integral team member in addressing an individual’s mental health, and when appropriate, substance abuse treatment needs.  It is the Blended Case Manager’s role to work together with the consumer to achieve his or her goals toward being as independent and self-sufficient as possible.  Ultimately, the Blended Case Manager and the consumer partner toward maximizing an individual’s quality of life while independently living within their own homes and communities.  


For individuals who are already receiving services at Lenape Valley Foundation, a referral to case management may be facilitated by consultation with their psychiatrist, therapist or any member of the Lenape Valley Foundation team.
