Blended Case Management

Blended Case Managers coordinate and connect children to services and community resources they may need to maximize their quality of life. Case Managers work with children and their families to provide the necessary resources and support so the child can reach his/her goals. Case Management Services are provided within the school, home, and community.

For people already receiving services at LVF, referrals to case management are made by your current staff person. For individuals not open for service at LVF, referral information can be obtained through Intake and Referral.
