Intellectual Disabilities

Intellectual Disability Supports Coordination is designed to assist individuals and their families in assessing current needs, developing Individual Support Plans, obtaining services and supports, locating, coordinating and monitoring those supports and services. Eligibility for ID services is based on a diagnosis of  Intellectual Disability prior to the age of 22.
