In July of 2009 Lenape Valley Foundation and the Area Agency on Aging launched a new collaborative service called SELF
(Senior Empowerment for Life Fulfillment).
 
This service specializes in providing support to individuals who are over 60 years old, who may have an unmet mental health need and a lack of adequate resources.  Individuals are seen in their own homes or the community with the goal to link individuals with appropriate mental health treatment near their home.  

The SELF program will provide education, case management, medication management, peer support and therapy until the individual is ready for a referral or linkage to another community provider.
