Assertive Community Treatment (ACT)

The TAY ACT team provides TAY (ages 18-25) individualized treatment that has a developmental focus to foster the acquisition and/or enhancement of skills needed to achieve greater self-sufficiency and confidence. The program recognizes and cultivates personal choice, social responsibility, and the developmentally appropriate need to find their own way and establish independence.

 

TAY ACT staff utilizes relationship development and person centered planning to assist the client in planning for their own future while collaborating with family members, community partners, and other identified supports throughout the process.

 

The opportunity exists to work with TAY in an independent living situation; as apartments become available and if an independent living situation would be a clinically appropriate intervention for the TAY’s individual recovery.

 

The TAY ACT team is implemented the same way as the traditional Adult ACT team; multidisciplinary team, community setting, and primary coordinator of services to those residing in Central and Upper Bucks County.  The TAY ACT program is also funded though medical assistance and the Bucks County office of mental health/mental retardation.
