Transitional Outpatient Services

The Transitional Outpatient Program is more intensive than traditional outpatient and is designed to achieve stabilization and resolution of psychiatric and chemical dependency conditions.  This level of care is specifically available for those adult clients who do not require the intense level of psychiatric care provided by inpatient or partial hospitalization programs.  Eligible participants may be experiencing acute symptoms of depression and/or anxiety and may be at risk for hospitalization, or may enter the Transitional Outpatient Program after discharge from an inpatient stay. 

 

The Transitional Outpatient Program offers a multi-modal, multi-disciplinary structure which includes individual therapy, group therapy, family therapy, psycho education, medication consultation, and discharge planning.  The Transitional Outpatient Program has a variable length of treatment and offers flexibility in scheduling as symptoms are alleviated and the individual is able to resume their typical activities.

 

Referrals may be made by individuals seeking treatment, physicians, therapists, or other health care professionals.
