Case Management

Case managers connect children to services, entitlements and other resources they may need to maximize their quality of life within their schools, homes, and communities. Case managers work with children and families to develop skills necessary to achieve and maintain successful social, family and/or school functioning.

For people already receiving services at LVF, referrals to case management are made by your current staff person. For individuals not open for service at LVF, referral information can be obtained through Intake and Referral.
