Intellectual Disability Services
Lenape Valley Foundation offers Supports Coordination to individuals with intellectual disabilities and Autism Spectrum Disorder who have enrolled in services through their county of residence. Our Supports Coordinators (SC) work to provide quality services in partnership with you, your family, team members, and chosen providers to assure that you or your loved one live a full and rich life in the community.
The Supports Coordinator’s role is to:
- Listen to what you need.
- Help you plan to meet those needs.
- Assist you in choosing services and supports.
- Ensure that the services identified in your plan are received at the amount and frequency that they have been approved.
- Monitor that services are appropriate and satisfactory.
- Make certain that health and safety needs are met./
YOUR SUPPORTS COORDINATOR CAN HELP:
- Locate and provide information on community resources such as utility assistance and food banks.
- Obtain information on respite options.
- Provide information on entitlement programs such as Medicaid, Food Stamps, Social Security, Medicare, and/or WIC.
- Make referrals and distribute information about programs for the elderly.
- Make referrals for vocational or employment programs through the Office of Vocational Rehabilitation.
- Assist you in locating behavioral health services.
- Provide information on social, recreational and summer camp options.
- Refer to the Administrative Entity for funded services when base funding is available.
- Provide information on home modification programs for individuals with physical disabilities.
- Advocate for you to move into the waiver program as there are openings or initiatives to meet needs.
- Attend IEP meetings with you and your family at your school.
Day-to-Day Assistance
Your SC is available to help you navigate the Intellectual Disabilities system, attend meetings with you and monitor the services you receive. While there may not be any funding available through the designated county entity, the SC will help you identify community supports that can help meet your needs.
If waiver funding becomes available, SCs schedule and go with you to tour provider agencies, such as day programs, community living arrangements, Lifesharing, or agencies that come to your home to help you increase daily living skills and address other areas of support that guide you in reaching your goals. SCs can also help you:
- Complete annual applications for Medicaid.
- Discuss problems or concerns with your services.
- Help you change service providers.
- Explain what type of funding you receive (if applicable).
- Clarify how that funding is used to acquire the services that you need.
Individual Support Plan (ISP)
Your SC will work with you to identify your vision for “Everyday Life” and create an Individual Support Plan (ISP) that addresses how to best support you. At least once per year, an annual ISP team meeting will be held to review your current needs and your current plan. We will then work with you to develop a new plan of needed services and support. At this important meeting, we can best help address your needs and implement services for you.
Monitoring
You may contact your SC any time you need to, but your SC will visit you at home or in a community setting at least twice a year to complete a monitoring form to ensure that your needs are being met and that you are healthy and safe in your environment. The SC is also required to follow up on any unusual incidents and any barriers to services and supports you or your family member may experience.
