SELF Program for Seniors
SELF (Seniors Empowered for Life Fulfillment)
In July of 2009 Lenape Valley Foundation and the Area Agency on Aging launched a new collaborative service called SELF (Seniors Empowered for Life Fulfillment). The SELF Program utilizes a recovery based approach in connecting older adults (60 years and older) with unmet behavioral health needs to services and supports that can assist them in addressing their goals. Individuals are seen in their own homes (across Bucks County) or in the community with the goal to link individuals with appropriate mental health treatment near their home. Services include but are not limited to:
- Connection to traditional behavioral health services for those for whom traditional services are acceptable and effective
- Time limited assistance and support to provide symptom relief and to regain the ability to function independently in the community
- Connection to and/or increase in natural supports such as collaborating with PCPs to aid in ongoing medication oversight and maintenance
The SELF program will provide education, case management, medication management, peer support and therapy until the individual is ready for a referral or linkage to another community provider. For more information and referral, call 267-893-5112.
