ACUTE PARTIAL HOSPITAL PROGRAM
The goal of the Acute Partial Hospital Program is to help participants understand their current problems and improve their coping skills. Through individual and group therapies, participants are encouraged to learn about their illness, practice practical skills to manage problematic behaviors, identify painful feelings that may inhibit progress, and improve interpersonal functioning.
ASSERTIVE COMMUNITY TREATMENT
ACT is an evidenced-based, nationally known service delivery model, providing comprehensive community based treatment for persons with severe and persistent mental illnesses.
Blended Case Management
Blended Case Management (BCM) services are designed to assist Bucks County residents with serious mental health issues to obtain needed mental health, medical, social, financial, vocational, educational and other necessary supports to maintain maximum independence and community functioning.
Court Ordered Evaluations
Lenape Valley Foundation has been providing forensic mental health services to the criminal justice system for over 25 years. We offer unique forensic services in Bucks County. These include evaluation of adult and juvenile offenders.

Forensic Response Team
Lenape Valley Foundation’s Forensic Response Team is comprised of experienced mental health professionals with extensive knowledge of the criminal justice system as it relates to individuals with behavioral health disorders, as well as treatment options and local community resources.
Outpatient Services
Lenape Valley Foundation’s Outpatient Department can help you or a loved one address these concerns and many others. We offer individual, family, and group therapy. Our psychiatrists and nurse practitioners are available to evaluate the need for medications and to prescribe and monitor the client’s response to them.
Peer Support Program
Peer support services are provided by adults with successes in recovery from mental health challenges. They often have broad experience with the mental health service system.
Residential Services
The Maximum CRR program is a licensed, recovery based setting that is transitional and rehabilitative in purpose. Residential staff members assist residents in building skills that are necessary to move to a more independent setting.
SELF Program for Seniors
The SELF program will provide education, case management, medication management, peer support and therapy until the individual is ready for a referral or linkage to another community provider.
Transitional Outpatient Services
The Transitional Outpatient Program is more intensive than traditional outpatient services and is designed to achieve stabilization and resolution of psychiatric and chemical dependency conditions. This level of care is specifically available for those adult clients who do not require the intense level of psychiatric care provided by inpatient or partial hospitalization programs.
Solution-Focused Brief Therapy
Solution-Focused Brief Therapy is an evidence-based practice of talking therapy for people seeking immediate change. The focus is on building upon current strengths and reaching goals. Sessions will concentrate on addressing the present and the future, not the past. This therapy promotes work outside of sessions and explores new ways to overcome challenges.
