Health Connections Nurse Navigator

The Health Connections Nurse Navigator is designed to give adults with a Severe Mental Illness (SMI) diagnosis and physical health conditions the extra support they may need when managing their physical and behavioral health.
This service provides a collaborative approach to improve physical and behavioral health factors. Services are delivered one-on-one, both in person and virtually, at the office, home,  and/or community.

How Can a Nurse Navigator Help?

  • Assist a person in identifying wellness needs & goals to develop a wellness plan
  • Help monitor vital measurements to assist in reaching wellness goals
  • Help educate individuals on their illness and learn how to manage it
  • Assist with tobacco cessation, nutrition plans, weight loss goals, wellness management, medication education, etc.
  • Link a person with community resources, treatment, wellness materials and accessing other health and wellness tools
  • Collaborate with an individual’s health care providers and case managers.
  • Help a person obtain needed specialists/appointments to improve overall health
