ACT Team Now Accepting Referrals – Immediate Openings Available

The Assertive Community Treatment (ACT) Team is currently accepting new referrals and has immediate availability for admissions. Pre-admission meetings can be scheduled within just a few weeks, offering timely access to intensive, community-based behavioral health support.

ACT is designed for individuals who benefit from a higher level of care than traditional outpatient or case management services can provide. If you are working with someone who may meet criteria, we encourage you to submit a referral.

How to Refer
To initiate the process, please submit a completed referral form to Program Administrator Ethan Hevener at ethan.hevener@lenapevf.org.

Eligibility Criteria
Individuals must:

  • Be 18 years of age or older
  • Reside in Central or Upper Bucks County
  • Have a primary diagnosis of Schizophrenia, Schizoaffective Disorder, or Bipolar Disorder
  • Demonstrate difficulty engaging with traditional outpatient or case management services, or require more intensive, community-based support

In addition, individuals must meet at least two of the following:

  • Two or more psychiatric hospitalizations within the past 12 months
  • Persistent or recurrent severe psychiatric symptoms
  • Co-occurring mental health and substance use disorders (lasting 6+ months)
  • Recent or high risk of involvement with the criminal justice system
  • Currently in a supervised or inpatient setting but able to live more independently with intensive support

Exclusion Criteria
ACT services are not appropriate for individuals whose primary diagnosis is:

  • Substance use disorder or substance-induced condition
  • Intellectual disability
  • Brain injury
  • A condition secondary to a medical diagnosis
